1. Scope

1.1 This Privacy Policy (the “Policy”) describes the practices of Millennium Health & Fitness, Inc. (“MHF”) with respect to information collected in connection with the website on which this Policy is posted (the “Site”). This Policy applies solely to the Site and forms part of the Terms of Use governing the Site.

2. Information MHF Does Not Collect

2.1 The Site does not offer user accounts, registration, or login functionality, and does not request personal information.

2.2 MHF does not collect, store, or process through the Site any information that identifies an individual personally, including name, email address, telephone number, mailing address, date of birth, Social Security number, employer, work location, health condition, medical history, or biometric identifier. The Site does not collect, store, or process protected health information as defined under the Health Insurance Portability and Accountability Act of 1996, as amended.

2.3 Because the Site does not authenticate users, MHF has no means of identifying which individuals accessed the Site or which Content any individual viewed.

3. Information Collected Automatically

3.1 MHF’s hosting provider maintains standard server logs for security and operational purposes. Such logs may record Internet Protocol address, browser type, device type, pages requested, and the date and time of each request. Log data is used solely to operate and secure the Site and to evaluate aggregate usage; it is not used to identify any individual and is not combined with any other data source for that purpose.

3.2 Log data is retained for ninety (90) days, after which it is deleted in the ordinary course.

4. Cookies

4.1 The Site does not use cookies or similar technologies to identify, profile, or track users. Any cookie set by the Site is strictly necessary for its operation and expires upon termination of the browser session. The Site does not use advertising cookies, third-party marketing cookies, web beacons, or cross-site tracking technologies.

5. Use and Disclosure of Information

5.1 MHF does not sell, rent, lease, or trade information collected through the Site, and does not sell or share personal information as those terms are defined under applicable state privacy law.

5.2 MHF may disclose log data only: (a) to its hosting and infrastructure providers, solely as necessary to operate and secure the Site; (b) as required by law, regulation, subpoena, or other lawful process; (c) as required under MHF’s contractual obligations; and (d) as necessary to investigate suspected security incidents or violations of the Terms of Use.

5.3 MHF may report aggregate, non-identifying usage counts to the organization sponsoring the wellness program the Site supports. Such reporting includes no information identifying any individual.

5.4 Certain Content is licensed from third-party providers and is delivered from servers controlled by MHF. No user identifiers are transmitted to those providers. This Policy does not apply to any third-party website to which the Site may link.

6. Security

6.1 The Site is transmitted over HTTPS using a current version of Transport Layer Security, and access to the underlying infrastructure is restricted to authorized MHF personnel. Because the Site collects no personal information, no personal information is exposed in the event of a compromise. No method of transmission or storage, however, is entirely secure, and MHF does not warrant the absolute security of the Site.

7. Children’s Privacy

7.1 The Site is intended for adults and is not directed to children under the age of thirteen (13). MHF does not knowingly collect information from children, and because the Site collects no personal information from any user, no such information concerning a child is collected or retained.

8. Changes to This Policy

8.1 MHF may amend this Policy at any time by posting a revised version to the Site. The “Last updated” date above reflects the most recent revision, and amendments are effective upon posting.
